Selkirk Neurology Expands Subspecialty Care Across the Inland Northwest

Selkirk Neurology has added a second movement disorder specialist, brought in remote monitoring technology to keep tabs on patients between visits, and expanded several of its specialty programs. Together, these changes grow what the practice can offer patients across eastern Washington, northern Idaho, and western Montana, and they reduce how often someone has to leave the region to find this level of care.

Based in Spokane, Selkirk Neurology is a multi-provider neurology group with a deep focus on movement disorders. A large share of its patients would otherwise drive hundreds of miles for subspecialty treatment. Closing that gap has been the point from the start.

Dr. Salil Manek, MD Joins the Practice

Dr. Salil Manek, MD has joined Selkirk Neurology, roughly doubling the practice's capacity for complex movement disorder care. He is the second movement disorder specialist on staff, and his arrival is the largest addition to the team since the practice was founded.

Dr. Manek trained through separate fellowships in neuromuscular and movement disorders, and he is a board-certified neurologist with decades of clinical experience. His work centers on Parkinson's disease, essential tremor, dystonia, ataxia, and related conditions, with a long track record of careful, patient-centered care in cases that are often difficult to manage. With two fellowship-trained specialists now in the building, the practice can put both of them on its hardest cases.

He is seeing new patients and accepting complex case consultations now.

A Growing Care Team

Dr. Manek joins Dr. Jason Aldred, MD FAAN, whose work spans movement disorders, electrodiagnostic testing, deep brain stimulation, and botulinum toxin therapy. The wider team also includes:

• Tiffany Raney, ARNP, a full-time movement disorders nurse practitioner who handles new patient evaluations, ongoing management, and care coordination. She is pursuing certification through the Association of Movement Disorder Advanced Practice Providers.
• Helena Maynard, RN, who runs the practice's Care Management program and is the day-to-day point of contact for many patients and families.

Remote Monitoring with StrivePD™

Selkirk Neurology has partnered with Rune Labs to offer StrivePD, an FDA-cleared platform built specifically for Parkinson's disease and movement disorder care. Patients wear a sensor that tracks motor symptoms between appointments, so the care team works from real data rather than relying only on what gets remembered and reported at a visit.

• Continuous tracking of tremor, motor fluctuations, and dyskinesia through a wearable sensor
• Objective data over time to guide medication changes, therapy adjustments, and levodopa pump or DBS programming
• Visibility into how a patient is doing between appointments, so the team can step in early
• A dedicated Care Management program so patients are supported in the weeks between visits, not just during them

The idea is straightforward: combine in-person subspecialty care with steady monitoring in between, and fewer problems go unnoticed until the next appointment.

HDSA Center of Excellence — Huntington's Disease

Selkirk Neurology is a Huntington's Disease Society of America (HDSA) Center of Excellence, one of a limited number nationally and the only one in the Inland Northwest. The designation reflects a sustained commitment to coordinated care for people and families living with Huntington's disease (HD).

• HD evaluations covering motor, cognitive, psychiatric, and functional health
• A working relationship with the University of Washington neurogenetics program
• Evidence-based management and access to current therapies
• Ties to HDSA's national resources, care networks, and family support
• Genetic counseling coordination, including support for at-risk family members

Deep Brain Stimulation (DBS) Program

Selkirk Neurology runs the only comprehensive DBS program in the Inland Northwest, working through an in-person committee that includes neurosurgery and neuropsychology colleagues. For patients with Parkinson's disease, essential tremor, or dystonia whose symptoms are no longer well controlled with medication, DBS can make a real difference.

• DBS candidacy evaluation and pre-surgical optimization
• Post-operative programming and long-term management
• Working relationships with neurosurgical partners for implantation
• Ongoing management, including troubleshooting and reprogramming

Botulinum Toxin Program

The practice's botulinum toxin program covers a range of neurological conditions that used to send patients to Seattle or farther. That care is now available in Spokane.

• Cervical dystonia and other focal dystonias
• Spasticity (post-stroke, MS, TBI, cerebral palsy)

EMG/NCV: Neurologist-Performed Electrodiagnostic Studies

At Selkirk Neurology, EMG/NCV studies are performed and read by a neurologist with more than 15 years of focused experience in neuromuscular disease and movement disorders, not handed off to a technician. That experience matters: subtle findings get caught, results are read in the context of the patient's full neurological picture, and reports come back quickly and in detail. The program handles everything from carpal tunnel syndrome and peripheral neuropathy to radiculopathy, myopathy, and motor neuron disease.

A Teaching Institution

Teaching is built into how the practice works. Every psychiatry and physical medicine & rehabilitation resident in the regional Spokane residency programs spends a month rotating at Selkirk Neurology, part of a broader effort to prepare the next generation of physicians for complex neurological care.
