Patients and Provider Referrals

 

Step 1

Discuss your care plan with your primary care provider or current specialist.

Step 2

Have your ordering provider fax a referral to our office. This needs to include the
diagnosis in question, any past neurological records including brain/spine imaging, labs, and
demographic sheet.

Step 3

Once the referral has been processed by our scheduling team it will be sent for
review to Dr. Jason Aldred. Once the review is complete and it is in the scope of our specialties, we will call you to schedule an appointment.

Selkirk Neurology is now using a third-party service for all medical records requests through ChartRequest. To get records, you must create an account by clicking the icon below.
